Common reasons
- Access token expired.
- Token belongs to wrong Meta app or business.
- Required permission is not approved or not available in current app mode.
- User role or business access is missing.
- Webhook verify token does not match.
Safe check
Re-test with the exact business, app, WABA, and phone number connected to BOS. Do not mix live and developer mode assets accidentally.
